In the request for company formation you should specify:
the jurisdiction of the future company
the type of company (LLC, LLP)
a description of the company's activities
the countries in which you plan to carry out business activities
the name of the company
And you will need to attach the following documents:
passport photo
selfies with passport
document confirming the place of residence
